Out-of-Network Benefits
Even if we are not in-network with your insurance plan, many patients still have out-of-network coverage available.
Depending on your plan, you may still receive reimbursement for medically necessary dermatology services.

Medical Dermatology Visits
New Patient Medical Visit — $200
Follow-Up Medical Visit — $125
Hair Loss Visits
New Patient Hair Loss Visit — $250
Hair Loss Follow-Up Visit — $175
Cosmetic Consultation
Cosmetic Consultation — $100
The cosmetic consultation fee may be applied toward a cosmetic treatment performed on the same day. If treatment is scheduled for a future date, the consultation fee is not applied toward the future treatment.

Shave Biopsy — $150
Punch Biopsy — $200
Additional Biopsy, Same Visit — $100 each
Intralesional Kenalog Injection — $75
Cryotherapy, 1–5 Lesions — $100
Cryotherapy, 6–15 Lesions — $150
Cyst Injection — $100
Acne Injection — $75
Nail Clipping / Fungal Testing — $75
KOH / Microscopy — $75
Other Procedures
Surgical excisions and other procedures vary based on the size, location, number of lesions, and complexity of treatment. Pricing will be discussed prior to treatment.
Biopsy & Pathology Fees
Biopsy pricing covers the procedure performed at Lux Dermatology and does not include laboratory or pathology interpretation fees. Biopsy specimens are sent to an outside pathology laboratory for evaluation. The pathology laboratory may bill you or your insurance separately for its services. Pathology charges are determined by the outside laboratory and are not controlled by Lux Dermatology.
Please note: Office visit fees do not include procedures, laboratory testing, pathology services, medications, or cosmetic treatments. These services may result in additional charges.
Cosmetic Treatments & Insurance
Cosmetic treatments are not covered by insurance plans.
Examples may include:
Botox / Dysport
Dermal fillers
Microneedling
PRP hair restoration
Chemical peels
Laser treatments
If a concern is considered medically necessary, portions of treatment or evaluation may qualify for insurance coverage depending on your plan.
Hair Loss Coverage
Insurance coverage for hair loss evaluations varies depending on the diagnosis and treatment plan.
Medical management of hair loss conditions may qualify for insurance coverage, while cosmetic or regenerative treatments such as PRP, compounded growth serums, and certain procedures are typically self-pay.
Our team will discuss all costs and treatment options with you before proceeding.

Copays, deductibles, and coinsurance are due at the time of service.
